This patient had hx of schezophrenia under medical treatment. He can not express
himself well due to medication. About one week ago, intermittent abdominal pain was
complained but his family did not care about it. However, poor appetite and fever
were noted in recent two days. He was sent to東勢榮民hospital and medication was prescribed first. However, the condition was deteriorated. Fever, tachycardia and low blood pressure were noted. Abdominal CT was done and a mass over gall bladder fossa was noted. He was trasnferred to ER on 98-3-6. At ER, septic shock status was noted and leukocytosis was also noted. Progressive dyspnea was also noted. F/u abd CT revealed GB emyema with liver abscess formation. GS was consulted and operation is indicated. AFter explanation the risk of operation, he was admitted for emergent cholecystectomy.After admission, emergent cholecystectomy was performed. Due to severe fatty liver and liver capsule tearing, liver packing was applied on 98-3-06. The packing was removed on 98-3-08 He was transferred to ICU for intensive care. His condition became stablized and endo was weaning and then transferred to ordinary ward. Persistent tachycardia was noted and CV was cosnulted. Besides, persistent bile leakage was also noted and only partial improvement was noted Thus. ERCP was arranged and cystic stump leakage was noted. ERBD was inserted. However, tubular adenoma of ampulla Vater was noted. In addition, PSY was consulted and the medication was adjusted. The drain was removed after bile leakage stopped. DM was noted and medication was applied. He was discharge under stable condition and will follow up in GSI PSY OPD. PPPD was suggested two months later. This time, he was admitted for PPPD intervention.
